- The github username you've used for the request
- The repository you were trying to import
- The exact timestamp (with time-zone)

GitHub account panicking, that correspond to my Company email. How reproduce:
Login in and try to associate your account and select any option. At time you verify it try to rollback to another option with cancel button to choose a different account association for your project and this point cancel always redirect to 404. There are only the confirm option left and no possibility to start back again or rollback to different account visibility scope. If it's not enough I can provide more information
